Our Home Care Team:
Administrator /Coordinator - Oversee the delivery of all staff and is responsible for managing the agency to include hiring and orientation of staff and completing assessment of all new customers for home care and other services that the agency offers. The A/C maintain and develop professional standards, procedures and policies for various institutional activities and ensure Activities for Daily Living (ADL's) are met. Is responsible for developing and expanding programs for community health and welfare.
Field Managers - Is responsible for field staff. Ensure that each worker provide appropriate service as assigned. FM's also oversee care delivery and customer outcomes to ensure that their needs are met. Being able to maintain compliance with local regulations and professional standards is an important aspect. Is responsible for all field training. FM's consult with customers to assess the quality of service that is required.
General Nurses - Our nurses provide various functions such as; scheduling assessment services and make recommendation base on their evaluation. They provide a full range of personalized care which may include instruction on chronic disease management and medication, pain management, wound care, IV infusion, and post-operative instruction. GN's provide the tools needed to ensure recovery. They also ensure individuals who are recovering from illnesses and accidents including follow-up care after discharge from a hospital, rehabilitation center, long-term care and facility before an aide/caregiver is assigned to their plan of care. They also provide nursing care within their scope of practice.
Nursing Assistants - Under direct supervision of GNs, they help clients get in and out of bed, walk, monitoring vital signs including temperature, pulse, respiration and blood pressure, turn and position, ROM, set up equipment, store and order supplies as needed. They observe their physical, mental and emotional conditions and report any change to the GN.
Personal Care Aides/Companions - Are responsible for daily living assistance such as but not limited to; bathing, grooming, dressing, laundry, toileting, shopping and errands, bed-making, food preparation as well as assistance with eating. They also provide light housekeeping, companionship and information reporting. In addition, they are able to aid with skin care.
General Aides/Personal Assistants - Responsible for the up keeping of the home and perform a wide range of services as requested by customers/clients (see the list of services for more details).
(Note: Nursing Assistants, Personal Care Aides and Companion must follow the care plan that is establish at initial home assessment and/or home assessment.)
